Sharing the experience of My First Trip in a Glider

One summer my family and I took a trip to Yosemite, via Napa Valley. An Australian friend of ours was in town at the time, and so we decided to take him on a little trip upstate. Our first stop was at Napa, where one day Bill (our Australian friend) took my sister and me for a ride in a glider.

I was fortunate in that I had the opportunity to go up in the air twice. Only two other people besides the pilot could ride in the plane so Bill, being the gentleman he was, gave up a turn, so that I could fly with him and also with my sister. The glider was attached to a regular plane at take off, and then, once we were airborne, the two planes separated. I could not believe how smoothly and how quietly the glider flew. I felt as if I were a giant bird, floating through the air at peace. I could not imagine being in any other spot rather than in the glider, looking down at the green vineyards below from the clear blue sky above. I think that we were up in the air for about thirty minutes, and then we landed. Bill got off of the plane, and my sister came onboard and that’s when the flight changed from tranquil to adventuresome for want of a better word.

Once we were airborne, the pilot asked Nancy (my sister) if she wanted him to do tricks with the plane. I was sitting in the back seat either saying “no” too quietly to be heard, or I was just thinking it to myself, while Nancy immediately responded “yes” with enthusiasm. Suddenly, the plane took a plunge downward, and then it went up rapidly and made a sharp left turn, while I was holding on for dear life, with my eyes closed. I thought the flight would never end, but it did eventually, as all good and bad things do come to an end eventually.

I forget where my dad was, but apparently we didn’t tell him that we were going to go up in a glider and, when he found out, my dad was furious with our friend for taking us up in the glider without his permission. I don’t think my dad ever openly became angry with Bill, however. The Ethical Involvement of Human Subjects in Research

When the controversial subject of human involvement in medical research arises, it can be met with an onslaught of ethical questions, considerations, and principles. Naturally, these points deserve our utmost attention, and it is our goal to establish a set of ideal (but attainable) goals in which human research may take place.

To answer the question, “what conditions should be met to assure ethical participation,” we have to understand the subjective nature of this term. What is ethical? Does a doctor have an ethical/moral obligation to take the interests of his patients over the general good of society? Or is it ethical, if not essential, to treat human beings as a means to a greater end, the antithesis of the Kantian model?

Conditions that dictate the ethical involvement of human beings in medical research should first and foremost focus on the relevence of the study. If there is in fact no material purpose for carrying out a study, and harm and pain are inflicted upon subjects, the issue arises of losing public trust in voluntary medical research. Eugene Passamani argues this point in his defense of randomized clinical trials; which at the same time is applicable to the question of ethical human research. Some research such as medical practice marketing requires extensive and accurate methodologies in order for it to have reliable results.

The utilitarian perspective argues that it’s not the act itself in question, but the outcome or result. If we apply utilitarian principle here, we can accordingly justify conditions in which informed consent isn’t a necessary condition on which human research is based. This steps outside principles outlined in past ethical codes, such as Nuremberg, but if it serves a greater good for society, ultimately yielding more utility, than the exclusion of voluntary informed consent is an ethical condition to which human research may adhere to.

The conflict with the utilitarian approach arises from a number of ethical views. The Kantian view asserts that it is never ethically licit to treat a human being (a rational moral agent) as a means to an end, regardless of what purpose it might serve for society. This is important to note because it draws in direct contrast to the utilitarian approach to medical research.

If we look at specific studies, such as the Willowbrook Hepatitis study, ew can see a two-fold approach and application of opposing ethical positions. Golby and Edsall, in their criticisms of the study, note that it was unethical to research on children in the first place. The question of whether it was also ethical to inject mentally retarded children (a substantially vulnerable population) with hepatitis is raised. Golby and Edsall go on to answer this portion of the question by asserting relatively the same principles.

The defense of such studies, again, focuses on the outcome of the research. Utilitarian principles justify such measures as using vulnerable populations and excluding complete informed consent (or consent under duress) when it yields such results as the Willowbrook study. Dr. Krugman also defended this study by suggesting that the children involved would’ve contracted hepatitis anyway, and probably received better care while participating in the study.

Thus far, we’ve established the two most important conditions which must be applied to determine the ethical participation of human subjects in medical research: significance of study; and overall benefit to society. Perhaps the most sensitive subject on this question is to what extent are doctors/researchers expected to convey honesty in their relationship to their patient.

Mack Lipkin asserts in his article that it is plausible to abstain from telling a patient the “whole truth” on the basis that such a notion doesn’t exist. However, this assertion can be taken to measures in which doctors may feel they have no obligations to disclose any information whatosever, thus creating a “slippery slope.” The matter of researchers and physicians outright lying to patients does not seem as pertinent as another issue in truth-telling: the question of placebo use.

Kant would argue against the use of placebos as a form of deception, as have others in the field of bioethics. However, placebos offer a control group in which researchers may better understand the results of tests involving new forms of treatment. Is it essential to always tell the truth in human research? I feel as if the situation isn’t as clear cut as the question implies. I support the initiative of clinical equipoise, in which researchers whom use a placebo don’t have firsthand knowledge of better treatment when conducting studies involving humans, for this would be a contradiction of the prima facie duty of a physician not to impose undue harm on a patient. However, there are also instances where this may not be an absolute.

Finally, and perhaps the most essential portion of my argument, is that researches involved in a study must not have a financial stake to be gained from the research. In the Willowbrook study, both Dr. Krugman and the University of Pennsylvania owned shares in the Genevo pharmaceutical company. This is clearly unethical as it draws a blurred line between performing research to benefit society, or the performance of research for monetary gain. This problem is indicative of the number of other moral/ethical issues that exist in a privatized health-care system, but we shall indulge in that argument at a later time. To formulate a condition which would constitute ethical research involving humans, all experiments should be undertaken by the government, and private institutions in medicine (forcing doctors and researchers to become employees of the state) should be abolished.

The conflicts that arise out of these positions are clear in either their utilitarian or deontological persuasion. Such conflicts are natural, but must be extinguished on material grounds that yield utility for all of society, while costs kept to a minimum. In conclusion, the conditions to which human research should adhere include significant purpose for performing a study; yield overall benefits to society; partial-honesty when applicable (includes placebo use); clinical equipoise (whenever possible); and nationalized research institutions. These are justified through utilitarian principles, and overcome the idealistic deontological approach through consequentialist considerations and the overall utility that may be yielded. Through such vigorous application, social utility will be yielded at minimal costs, thus meeting the inherent condition of medical research in the most basic sense.

Since I hadn’t been happy, with the cheap CVS disposable razors, I figured I would try this one out. The trockenrasierer is one of them that are a electric razor for shaving the hairs of the girls. The skin tone will not be affected through the use of the razors.

Upon opening it I had a gray and pink handle, 2 cartridges, and a shower hanger. The first thing I liked about it was that the handle was sturdy, was rubber, and had a good grip to it. All I had to do was put the cartridge on the top of the handle, press down on the button, and pull it up. Then I had my razor and was ready to go to work with it. It does have a plastic safely cap to put over it, when it is not in use.

What I like about this one, is that the cartridge has 4 blades, and on either side of them, strips that contain aloe and Vitamin Complex. Even though I purchase a good quality shaving gel, it is still nice to know that these strips should keep my shaved areas moisturized.

So I got in the shower, and applied shaving gel under my arms. I then brought down the razor 3 or 4 times, under one arm, and then rinsed the razor. I checked under my arms and the hairs had been removed. I did the same under the other arm.

Next came the bikini area, and because I have done this for 50+ years, there is always dark stubble. If my razor isn’t a good quality one, then this is the area, that I have a problem with. I again put the gel on, and shaved as normal. I am pleased to say that it worked fine on this part of my body.

Now on to my legs. I made sure that it had been a week since I last shaved them. Normally I do it every 2 to 3 days, so there was plenty of hair. I used long strokes and the Schick Quattro High Performance Razor did it’s job. I had really smooth legs. They felt soft and didn’t need moisturizing after-wards.

Once finished I let the razor dry and then put the plastic safety clip on it.

I have used this razor 4 times now and it is still working just as good as the first time. Having 4 blades, I feel that it gives a better shave, then many of the other razors I have used. My only concern will be when I have to purchase refill cartridges. That is where the companies make their money.

This came with a plastic shower hanger, but I didn’t use it, as I like to dry my razor, and store it away from the shower. I would think it could go moldy if left in the shower itself.

Although I didn’t need to put a new cartridge in, I took off the original cartridge, just with a push on the button, and it was so simple. Putting a new one in is a simple task.

Spinal Meningitis: Symptoms, Diagnosis and Treatment Options

The contraction of spinal meningitis is considered a medical emergency for two reasons: 1. It has a high mortality rate if gone untreated, and 2. The symptoms are extremely painful and can lead to secondary defects that can be life threatening.

Meningitis is characterized by the inflammation of the membranes that cover both the brain and the spinal cord. Bacteria, viruses, fungi and parasites are the most common causes, though there have been reports of chemically-induced meningitis. The symptoms of spinal meningitis include high fever (over 100 degrees), severe headaches, dementia and stiffness of the neck.

At first, spinal meningitis might feel like the flu. Fever, chills and lethargy are common initial symptoms, and if a patient feels that he or she only has a common cold or the flu, treatment might not be sought immediately. A stiff neck and photophobia should soon follow, however, and these are classic symptoms of spinal meningitis.

 

The possibility of spinal meningitis is taken very seriously, and a physician will first order a lumbar puncture (spinal tap) so that he can microscopically examine cerebrospinal fluid for a complete blood count with differential. He will also perform a chemical analysis and bacterial cultures.

Because spinal meningitis affects the brain as well as the spinal cord, neurological defects are common. If there is any sign of intracranial pressure, the physician will order a CT scan of the cranium to try and locate any points of brain herniation or brain damage.

In all cases of spinal meningitis, the patient must be hospitalized for close observation and to administer medication. For bacterial meningitis, antibiotics will be given orally at first, and then delivered intraveinously until the worst of the symptoms pass. Antibiotics will be given even more regularly if there is a possibility of intracranial pressure from the swelling of tissues.

Viral meningitis, on the other hand, cannot be treated with antibiotics. In most cases, viral meningitis will not be as severe as bacterial meningitis, and patients will recover much more quickly. If there is a possibility for seizures, the physician may prescribe anticonvulsants such as phenytoin to counteract that possibility, and corticosteroids may be necessary to guard against brain inflammation.

There is a cause for alarm is the patient has been in recent, close contact with other people, such as at school or summer camp. Spinal meningitis can spread rather quickly, and students or adults in close contact with others should be monitored closely for signs of the disease. In order to prevent future illness, students or adults who have been exposed to spinal meningitis will be given a prophylactic treatment of antibiotics. Such drugs may include rifampicin, ceftriaxone or ciprofloaxacin.

Pregnant women who contract spinal meningitis may have to stay in the hospital until she has carried the child to term. An intense barrage of antibiotics at birth may keep the infant from developing spinal meningitis as well, and proper pre-natal care will lessen the chances of the child being born with birth defects. Women with spinal meningitis should be given ceftriaxone rather than any other type of antibiotic, as drugs of any kind are dangerous during pregnancy.

Vaccinations have been developed for certain strains of spinal meningitis, though some are more effective than others. Specific vaccinations for meningitis prevelant in Norway and Africa have been given to great success, and vaccinations for childhood meningitis have also been quite effective.
